The Cholangiopancreatographic and Pathologic Findings of Adult Choledochal Cysts

Abstract
Choledochal cyst has been known to be associated with an anomalous junction of pancreatobiliary ductal system.
We compared ERCP findings of five adult cases of choledochal cyst with those of nine normal persons, in respect to the length and diamether of common channel, ad the angle of the pancreatobiliary junction. All patients with choledochal cysts had
long
and ectatic common channel(P<0.05). Four cases of choledochal cysts were compatible with B-P type and cystic type, one case with P-B type and cylindrical type by the classification of Kimura and Okadarespectively.
In morphologic features, fibrotic and/or mild glandular patterns were revealed in four cases of cystic type, ad severe glandular pattern in one of cylindrical type. This finding was very similar to Okada's report, except that the fragmentation of
elastic fibers was observed in all cases.
In conclusion, long, ectatic common channel and anomalous junction of pancreatobiliary ductal system are important components in causative aspect of choledochal cyst. Though some differences are existed between child-onset and adult-onset
choledochal
cysts, adult-onset our result and other reports are reviewed.
